Licensed Practical Nurse
We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our team!. The LPN is responsible for assessing participant health needs, screening for withdrawal or mental health issues, coordinating medical care, and ensuring safe, supportive recovery services while supporting continuity of care in collaboration with physicians, nurse practitioners, and other providers.

Responsibilities
- Floating Shifts - Days (7AM to 3PM), Evenings (3PM to 11PM) and Nights (11PM to 7AM)
- Provide comprehensive participant care, including admission health assessments.
- Accurately document all medical assessments, interventions, and outcomes.
- Serve as the shift lead for medical operations and care delivery.
- Monitor vital signs and conduct ongoing health evaluations.
- Screen for withdrawal severity (e.g., CIWA assessments) and intoxication risk.
- Conduct physical assessments: bruising, rashes, injuries, or bone deformities.
- Administer breathalyzer tests as needed.
- Screen participants for immediate psychiatric needs.
- Provide wound care (cleaning, antiseptic, covering, infection monitoring).
- Administer and document medications accurately and promptly.
- Monitor progress and determine whether participants can be safely treated in the facility.
Team Support
- Provide training and guidance for Health Techs/Recovery Specialists.
- Ensure safe, clean, and supportive working conditions.
- Serve as backup to the Program Manager when needed.
Quality Assurance
- Maintain accurate and up-to-date EHR records.
- Ensure risk assessments, ER discharges, and medical clearances are uploaded daily.
- Monitor for documentation accuracy, compliance, and confidentiality.

Qualifications
- Current New Mexico LPN license (required to legally practice).
- CPR/First Aid/AED certification.
- Valid New Mexico driver’s license and clean driving record.
- Previous experience in substance use, mental health, or recovery settings preferred.
- Demonstrated ability to take on increasing leadership and shift responsibility.
