Nurses needed to visit patients: Private Duty
Responsibilities:
Provide nursing care to elderly, Children and/or disabled patients
Assess patient needs and develop corresponding treatment plans
Monitor patient status and notify office staff and supervising physician of significant changes
Communicate with supervising physicians and other members of the care team to provide effective care
Educate patients, family members, and caregivers on their conditions, diagnoses, medications, and treatments
Ensure patient records are accurate and complete
Serve as a patient advocate and provide advice and support to the patient and his/her family
Record a patient’s medical history accurately
Monitor patients’ condition including fluid intake and output and compose patient charts
Provide emotional and psychological support when needed
Communicate with patients’ family or friends to provide advice, comfort and release instructions
Qualifications:
Previous nursing experience or Entry-level
Strong communication skills and ability to teach
Must be trustworthy, friendly, and dependable
Strong understanding of nursing, medical skills, and care plans
Must be self-motivated and able to work well independently
MUST HAVE A VALID ILLINOIS LICENSE FOR LPN
Proficiency in Spanish Language an advantage
Skills
Proven experience as LPN or similar role or Entry-level
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of medical and hospital terminology
Good physical condition and stamina
Excellent communication and interpersonal skills
Compassionate and able to handle stress
Successful completion of LPN licenses program is a must
