    Job Type: Full Time Day Shift 6p-6a (2 Positions Open)

    Job Summary:
    We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Tri-Community Nursing Center. The LPN will provide high-quality direct patient care and support to our nursing staff under the supervision of a Registered Nurse (RN) and physician. The ideal candidate will possess excellent interpersonal skills, attention to detail, and a strong desire to make a positive impact on the lives of our residents.

    Key Responsibilities:
    - Provide direct patient care, including administering medications and treatments as prescribed by the healthcare provider.
    - Monitor and record vital signs, patient health status, and changes in condition.
    - Assist patients with daily living activities, including bathing, dressing, and meals.
    - Maintain accurate and up-to-date patient records and documentation.
    - Collaborate with RNs, physicians, and other healthcare professionals to develop and implement patient care plans.
    - Educate patients and their families about health conditions, treatment options, and preventive care.
    - Ensure a safe and clean environment for patients, adhering to infection control protocols.
    - Respond to patient calls and address their needs and concerns promptly.
    - Assist in the orientation and training of new nursing staff or student nurses as needed.
    - Participate in team meetings and contribute to quality improvement initiatives.

    Qualifications:
    - Valid and active LPN license in Louisiana.
    - Completion of an accredited practical nursing program.
    - Previous experience in a healthcare setting preferred, but not required.
    - Strong communication and interpersonal skills.
    - Ability to work independently and as part of a multidisciplinary team.
    - Compassionate, patient-centered approach to care.
    - Excellent organizational and time management skills.

    Physical Requirements:
    - Ability to lift, move, and assist patients as needed.
    - Ability to stand, walk, and perform physical tasks for extended periods.
    - Capacity to manage the demands of a fast-paced healthcare environment.
