Pediatric LPN – Evening Home Care

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled, one-on-one care for a pediatric patient in a home setting. The patient requires tracheostomy (trach) care, and this role is perfect for LPNs who want a consistent evening schedule and the opportunity to make a meaningful impact.

Key Responsibilities

Provide nursing care based on the patient’s approved plan of care

Assist with personal care and daily activities

Maintain a safe, clean, and supportive home environment

Observe, document, and communicate any changes in patient condition

Follow all documentation and record-keeping requirements

Qualifications

Active Ohio LPN license

Experience with trach care required

Current CPR certification

Valid driver’s license and reliable transportation

Recent TB test or chest X-ray

Ability to pass background and drug screenings
