Your Role:

• Provide compassionate, high-quality care to residents
• Administer medications and treatments according to plan of care
• Monitor and document patient progress and response
• Communicate effectively with nursing staff and interdisciplinary team members
• Ensure all care is delivered in compliance with facility, state, and federal regulations

Qualifications:

• Active New York LPN License
• Strong clinical skills and communication abilities
• Team-oriented with a passion for excellent patient care
• Prior experience in a skilled nursing or rehab setting preferred
