CLF Consultants LLC seeks Licensed Practical Nurses to deliver skilled nursing care at the VA Hudson Valley Health Care System across its Montrose and Castle Point campuses in New York's lower Hudson Valley. The LPN provides hands-on bedside care to Veterans in the Intermediate/Medical Nursing Home Care Unit and to psychiatric patients, working under the supervision of the Nurse Manager and alongside the facility's clinical teams. This is a mission-driven federal healthcare role serving an adult Veteran population spanning medical, geriatric, and behavioral health needs. The two campuses sit about 25 miles apart in Westchester and Dutchess counties. CLFC supports each nurse through VA credentialing, onboarding, and mandatory training coordination so the clinician can focus on patient care from the first shift.
Schedule: Four shift patterns (Eastern Time): Day 7:30 AM to 4:00 PM; Evening 3:30 PM to 12:00 AM; Extended Day 7:30 AM to 8:00 PM; Night 7:30 PM to 8:00 AM. Rotations include weekends and federal holidays.
Key Responsibilities
- Deliver skilled nursing care to adult medical, geriatric, and psychiatric patients in the Intermediate/Medical Nursing Home Care Unit and other assigned units
- Prepare, administer, and accurately document oral, topical, subcutaneous, intramuscular, and intravenous medications in accordance with facility policies
- Observe and document each patient's response to medications and report changes in condition to the RN or physician
- Perform vital signs monitoring, specimen collection, wound care, and related bedside procedures within the LPN scope of practice
- Support patient and family education and reinforce care instructions under RN direction
- Document care in the VA electronic health record in accordance with facility standards
- Apply facility safety standards and infection control practices; complete and keep current all VA mandatory training
- Float between the Montrose and Castle Point campuses as assigned
Required Qualifications
- Graduate of a state-approved school of practical or vocational nursing
- Active, current LPN license issued by any U.S. state, territory, Commonwealth, or the District of Columbia, in good standing and free of restrictions
- Current CPR/BLS certificate issued by the American Heart Association
- Demonstrated competence caring for adult psychiatric, medical, and geriatric patients
- Medication administration proficiency across oral, topical, subcutaneous, intramuscular, and intravenous routes (IV duties only as authorized by license, documented competency, VA policy, and supervising RN direction)
- Fluency in speaking, understanding, reading, and writing English
- Ability to clear federal credentialing, background and exclusion screening, and pre-placement health requirements
- Availability to rotate across shift patterns, including weekends and federal holidays, with reliable transportation to both campuses
Strongly Preferred Qualifications
- New York State LPN licensure
- Experience in long-term care, skilled nursing, community living center, or nursing home care units
- One year of clinical LPN experience in acute care, long-term care, or behavioral health (exceptional new graduates with relevant rotations may be considered)
- Behavioral health or psychiatric nursing experience with adult populations, including prior care of Veterans
- Prior VA, military, or other federal healthcare experience, including familiarity with VA CPRS or other EHR systems
- Documented IV therapy competency; geriatric, dementia, and memory care experience
Compensation and Benefits
- $67,184 to $81,328 annual ($32.30 to $39.10 per hour), commensurate with experience, W-2 full-time with benefits
- Additional $2.50 per hour shift differential for evening and night assignments; overtime and federal holiday pay per the assignment schedule
