Your Role: Licensed Practical Nurse (LPN)
As a licensed practical nurse under the nurse manager's or designee's direction, the nurse provides professional and skilled nursing care for individuals with developmental disabilities in the office, homes and community settings. You’ll encourage independence, promote engagement, and help clients achieve their personal goals with compassion and integrity.
Key Responsibilities:
Provide direct nursing care under RN supervision.
Perform G-tube feedings, flushes, and site care; monitor for intolerance or infection.
Implement and support special diet plans as ordered.
Perform wound care and dressing changes per care plan.
Perform nursing respite.
Monitor blood glucose and observe for signs of hypo/hyperglycemia.
Assist with toileting, bowel and bladder routines, and hygiene as needed.
Monitor mobility and safety; report falls or injuries.
Observe and report changes in condition to the RN.
Reinforce caregiver education on tube feeding, diet, and basic diabetes care.
Document care provided and patient response.
Document all nursing care and patient response.
Chart behavior or condition observations on the computer
Drive or assist the driver in safely transporting individuals with disabilities, ensuring comfort and support during transit
Ensure safety and dignity in every interaction
Carry out additional duties as directed by the supervisor
Required Qualifications:
Must be 18+ years old
Active LPN license
Home health experience preferred
Experience with diabetes and enteral feeding preferred
Comfortable with telehealth
CPR and First Aid Certification
Article 9 Certification (or willingness to obtain)
Prevention & Support Certification (or willingness to obtain)
Clean Level One Fingerprint Clearance Card (or ability to obtain)
Able to pass a criminal background check
Reliable transportation and willingness to travel locally
Nice to Have:
Experience working with individuals with developmental or intellectual disabilities
