    Schedule: Day Shift: 8:00 AM – 4:30 PM Rotates every 4th weekend and holiday. Position Overview: EverHeart Hospice is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our dedicated care team. In this role, you will provide high-quality, hands-on patient care and support to individuals and families during daytime hours. Responsibilities: Responsibilities of the Position: • Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including: • Routine Home Care Visits • End of Life / Continuous Care • Referrals • Care Center Assistance • Chart Reviews • Other duties as assigned • Recognize and promptly report any changes in the patient’s mental and/or physical status to the registered nurse, both verbally and in writing when appropriate • Explain hospice services and benefits related to Medicare, Medicaid, and private insurance; understand when to implement Palliative Care interventions and use ABNs • Obtain and enter orders from Medical Directors and Physicians when appropriate • Order necessary Durable Medical Equipment (DME) for patients. • Facilitate continuity of care by coordinating the admission of home-care patients to inpatient settings (hospital, nursing home), as needed. • Work evenings and weekends as needed to meet patient, family, and agency scheduling needs • Complete all documentation in accordance with agency policy, accreditation standards, and governing requirements • Maintain clinical competency for assigned responsibilities and procedures • Perform other duties as assigned Qualifications: Education and Experience: • Graduation from an approved School of Nursing • Current LPN licensure in the State of Ohio • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing Special Skills and Requirements: • Shall have a valid driver’s license • Excellent customer service skills • Detailed with excellent communication, documentation, and organization • Ability to follow instructions • Ability to work independently without supervision • Travel to other sites as needed. Travel may be in inclement weather • Ability to lift 25 lbs of supplies or equipment • Requires various amounts of sitting, standing, twisting, and turning, bending, lifting, and walking • Ability to assist with patient care activities such as ambulation, transfers, and positioning
