Compensation: $30 - $35 Hourly

About the Job
The Licensed Practical Nurse (LPN) will serve as a key point of contact for patients managing chronic conditions such as diabetes, hypertension, and heart disease. This role focuses on patient outreach, clinical follow-ups, care plan adherence, and coordination with providers to ensure consistent care delivery. The position is full-time and onsite in Lakewood, NJ, with responsibilities that include both in-office support and remote patient engagement.
Duties and Responsibilities:
Conduct monthly CCM check-in calls with patients
Monitor patient symptoms, medications, and care plan adherence
Document all interactions accurately in EMR systems (PCC preferred)
Coordinate with physicians, RNs, and care teams for follow-ups
Track labs, screenings, and preventive care requirements
Ensure compliance with CMS CCM documentation and time tracking guidelines
Identify patient risks or red flags and escalate appropriately
Support in-office coordination when required
Qualifications :
Active LPN license (U.S.)
1–3 years of experience in care coordination, telehealth, or clinical support
Experience with EMRs (PCC strongly preferred)
Strong communication and patient engagement skills
Comfortable working with elderly and chronic care patients
Understanding of HIPAA compliance and patient confidentiality
Ability to balance clinical judgment with administrative documentation
