    About the Job

    The Licensed Practical Nurse (LPN) will serve as a key point of contact for patients managing chronic conditions such as diabetes, hypertension, and heart disease. This role focuses on patient outreach, clinical follow-ups, care plan adherence, and coordination with providers to ensure consistent care delivery. The position is full-time and onsite in Lakewood, NJ, with responsibilities that include both in-office support and remote patient engagement.

    Duties and Responsibilities:

    • Conduct monthly CCM check-in calls with patients

    • Monitor patient symptoms, medications, and care plan adherence

    • Document all interactions accurately in EMR systems (PCC preferred)

    • Coordinate with physicians, RNs, and care teams for follow-ups

    • Track labs, screenings, and preventive care requirements

    • Ensure compliance with CMS CCM documentation and time tracking guidelines

    • Identify patient risks or red flags and escalate appropriately

    • Support in-office coordination when required

    Qualifications :

    • Active LPN license (U.S.)

    • 1–3 years of experience in care coordination, telehealth, or clinical support

    • Experience with EMRs (PCC strongly preferred)

    • Strong communication and patient engagement skills

    • Comfortable working with elderly and chronic care patients

    • Understanding of HIPAA compliance and patient confidentiality

    • Ability to balance clinical judgment with administrative documentation
