Position Overview: PHC is urgently seeking a caring and skilled LPN to join our team! In this role, you’ll provide hands-on patient care, assist with clinical procedures, and support our providers to ensure smooth, high-quality care delivery.

Key Responsibilities:

Take vitals, collect patient histories, and prepare for exams

Administer medications and assist with procedures (EKGs, injections, GYN exams, etc.)

Educate patients on medications and care plans

Coordinate referrals, follow-ups, and medication refills

Maintain clinical supplies and follow HIPAA/OSHA guidelines

Support PCMH initiatives and participate in quality improvement

What You’ll Bring:

Active LPN license and strong clinical skills

Experience with EHR systems preferred
