Job Title: LPN / LVN – Long Term Care

Location: Watertown, NY

Shift: 8 Hour Days

Position: LPN / LVN

Hours: 40 hours week

Schedule: Deferred schedule, no set schedule available

Start/End Date: 04/13/2026 – 07/15/2026

Radius Rule: 100-mile radius rule applies

Department: Nursing Float (Long Term Care)

Responsibilities:
Provide direct patient care in a long-term care setting
Administer medications and treatments as ordered
Monitor and document patient conditions and changes
Assist with daily living activities and patient support
Work with patients with dementia, chronic diseases, and diabetes
Utilize PointClickCare for documentation

Requirements:
1–3 years of LPN experience required
SNF or LTC experience accepted
Active New York LPN license required
Ability to work 8-hour day shifts

Certifications:
BLS (AHA)
