The LPN will assist the Registered Nurse with planning, coordinating, and managing the care services and act within the state and federal regulations. The LPN is responsible for the overall direction, coordination, and monitoring of direct care service delivery under the direction of the Registered Nurse. Responsibilities: • Lead and coordinate daily nursing activities to ensure high-quality care for our residents. • Collaborate with the Registered Nurse to develop and implement personalized care plans. • Monitor and document residents' health status, ensuring timely interventions when necessary. • Provide compassionate support and guidance to residents and their families, fostering a warm community. • Supervise and mentor nursing staff, promoting a culture of continuous learning and improvement. • Ensure compliance with state and federal regulations, maintaining the highest standards of care. • Act as a liaison between residents, families, and healthcare providers to facilitate effective communication and care coordination. Qualifications: Requirements: • Must be licensed with the State of Wisconsin or Minnesota as an LPN, and must maintain current unencumbered licensure. • Previous experience in a related field. • Maintains current background check on file with the State of Minnesota or Wisconsin and agencies involved. • Must have a valid Minnesota or Wisconsin Driver’s License. • Drives company and personal vehicle in a safe manner and in accordance with MN and WI State Laws. • Must be able to read, write, and communicate in English. Compensation: $30 - $33 hourly
