The LPN will assist the Registered Nurse with planning, coordinating, and managing the care services and act within the state and federal regulations. The LPN is responsible for the overall direction, coordination, and monitoring of direct care service delivery under the direction of the Registered Nurse. Responsibilities: • Lead and inspire a team of caregivers to deliver compassionate, high-quality care to our residents. • Coordinate with the Registered Nurse to develop and implement personalized care plans that meet each resident's unique needs. • Monitor and evaluate the effectiveness of care services, ensuring compliance with state and federal regulations. • Foster a positive and supportive environment by addressing concerns and providing constructive feedback to team members. • Maintain accurate and up-to-date medical records, ensuring confidentiality and adherence to healthcare standards. • Facilitate open communication with residents and their families, providing updates and addressing any questions or concerns. • Identify opportunities for process improvements and collaborate with the team to implement innovative solutions that enhance care delivery. Qualifications: • Experience in a healthcare setting, preferably in elder care, to ensure compassionate and effective service delivery. • Ability to lead and inspire a team, fostering a positive and supportive environment for both staff and residents. • Proven track record of coordinating with healthcare professionals to develop and implement personalized care plans. • Strong communication skills to facilitate open dialogue with residents, families, and team members. • Commitment to maintaining accurate and confidential medical records in compliance with healthcare standards. • Ability to identify opportunities for process improvements and collaborate on innovative solutions. • Knowledge of state and federal healthcare regulations to ensure compliance and high-quality care delivery. Compensation: $30 - $33 hourly
