Beautiful Minds Group is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team! This is a dual-role position supporting individuals with behavioral needs through in-home care and direct support services.

We are looking for nurses who are patient, reliable, and passionate about making a meaningful impact in the lives of youth and young adults.

Position Overview

As an LPN with Beautiful Minds Group, you will provide hands-on nursing care while also supporting daily living needs in a home-based setting. This role blends clinical responsibilities with direct support, offering a unique opportunity to build strong, meaningful relationships with the individuals we serve.

Qualifications

Active Licensed Practical Nurse (LPN) certification (required)

1–3 years of experience preferred (new graduates are welcome to apply!)

Experience working with behavioral youth (HIGHLY preferred – bonus consideration)

Previous experience as a Direct Support Professional (DSP) or Home Health Aide (HHA) is a plus

Must be able to pass all pre-employment screenings and background checks

Valid driver’s license and reliable transportation required

Key Responsibilities

Provide in-home nursing care and monitor client health

Assist with activities of daily living (ADLs)

Support individuals with behavioral and emotional needs

Administer medications and follow care plans

Maintain accurate documentation and communication with the care team

Ensure a safe, supportive, and structured environment
