Job Title: Licensed Social Worker (LMSW/LCSW)

Skills/Experience
- Conduct timely and comprehensive biopsychosocial assessments for hospitalized Veterans.
- Collaborate with medical, nursing, and allied health staff to develop individualized, goal-driven discharge plans.
- Identify and address social determinants of health that may impact length of stay, readmission risk, or discharge safety.
- Perform and document mental health evaluations, suicide risk screenings, and crisis interventions.
- Lead or participate in interdisciplinary rounds to coordinate care and communicate critical updates.
- Educate patients and families on treatment options, prognosis, and available VA and community-based resources.
- Facilitate goals-of-care conversations and assist with advance directive completion and life-sustaining treatment decisions.
- Coordinate psychiatric inpatient admissions and manage civil commitment petitions when indicated.
- Address housing instability, elder abuse, or neglect in collaboration with community resources and Adult Protective Services.
- Arrange placement in post-acute care settings, including rehabilitation, long-term care, hospice, or residential programs.
- Initiate and oversee home health, homemaker/home aide, or telehealth referrals to ensure continuity post-discharge.
- Track and manage cases that require ongoing post-discharge follow-up or transitional case management.
- Ensure documentation in the electronic medical record is accurate, timely, and reflects care provided and plans made.
- Respond to medical emergencies as part of the inpatient team, supporting psychosocial needs during crisis events.
Required Skills
- Minimum of two years of current professional social work experience
- Demonstrated experience providing services in an inpatient or acute care setting
- Proven experience in case management, including discharge planning and care coordination

- Strong crisis intervention and mental health assessment skills.
- Expertise in discharge planning and connecting Veterans with appropriate resources.
- Proficiency in interdisciplinary collaboration and patient advocacy.
- Commitment to maintaining patient privacy and respecting individual rights.
- Competence with electronic medical records and documentation protocols.
Degree Requirements
- Minimum: Master of Science in Social Work (MSW) from a nationally accredited school.
- Licensed Clinical Social Worker (LCSW) is preferred.
Certification
- LMSW or LCSW
- Licensure: Current, full, and unrestricted social work license in any U.S. State, Territory, or the District of Columbia.
- Certification: Basic Life Support (BLS) certified by an American Heart Association (AHA)-approved vendor
- National Provider Identifier (NPI) number
Salary: $42/hr.
