Compensation: $60,000 - $80,000 Annual Salary

We are looking for a Licensed Social Worker to support residents and families at our skilled nursing facility in Ellicott City, MD. This is a full-time, on-site role handling psychosocial assessments, care planning, discharge planning, and resident and family support as part of the interdisciplinary team.

Qualifications:

\- Active Maryland social work license (LMSW, LGSW, or LCSW-C)

\- Experience in skilled nursing, long-term care, or a healthcare setting preferred

\- Knowledge of psychosocial assessment, care planning, and discharge planning

\- Strong communication and documentation skills

Responsibilities:

\- Complete psychosocial assessments and MDS social work sections

\- Develop and update care plans with the interdisciplinary team

\- Coordinate discharge planning and community resources

\- Support residents and families through admission, stay, and transitions

\- Maintain accurate documentation and regulatory compliance
