    About the Role:

    Join BrightStar Care of Conejo Valley and Coastal Los Angeles as a Licensed Vocational Nurse and provide compassionate one on one care to patients of all ages.

    Shift Details:

    2:30 PM to 11:00 PM

    3 Day a week scheduling

    1:1 Elder Care

    Shift Rounds

    Client Location: Simi Valley

    Requirements:

    Current California LVN license.

    1 + years experience in home health, hospice, or in-home care preferred.

    1+ years experience caring for adults with physical and cognitive disabilities.

    1+ year experience providing catheter care, maintenance, monitoring, and infection prevention in a clinical or home health setting.

    Compassionate, dependable, and patient-focused.

    Basic Life Support (BLS) certification required.

    Ability to work flexible hours, including evenings and weekends.

    Reliable transportation and valid driver’s license.

    Responsibilities:

    Perform and manage catheter care, including maintenance, monitoring, and infection prevention

    Provide skilled nursing care in patients’ homes across pediatric, adult, and geriatric populations

    Administer medications, treatments, and therapies as prescribed by the physician

    Monitor and assess patient health status, vital signs, and clinical progress

    Collaborate with patients, families, and interdisciplinary care teams to develop and implement individualized care plans

    Educate patients and families on disease processes, medication management, catheter care, and home care techniques

    Respond promptly and professionally to changes in patient condition and emergency situations

    Maintain accurate, timely documentation in electronic medical records (EMR)

    Participate in ongoing training and professional development to maintain and enhance clinical competency
