Job Description Summary
Supervised By: Registered Nurse The Licensed Vocational Nurse provides direct care to home health patients based on an established plan of care created by the registered nursing in collaboration with the IDT. Care is provided under the direct supervision of a registered nurse in collaboration with the IDT and comprehensive plan of care to provide safe and effective patient care.
Roles & Responsibilities
Provides direct care to patients following the home health plan of care
Submits accurate and timely nursing follow up assessment notes to organization
Reports to RN any changes in patient’s condition
Provides routine and continuous level of care as ordered by IDT
Performs skilled instruction to patients and families regarding medication administration, management, and treatments
Provides patients and families with emotional support and therapeutic communication
Encourages patients and families to participate in the home health plan of care
Provides patient education regarding disease process, s/s of exacerbation and measures to promote comfort
Coordinate care with other home health care providers including nursing, therapy and supportive services, such as CHHA
Provides safe medication administration, reconciliation and home oxygen therapy management and education
Performed duties as assigned by the registered nurse timely and accurately
Attends monthly in-services and professional development opportunities
Follows all agency policies and procedures, including in Infection Control, Emergency management, medication management, emergency management, pain management and overall patient care policies and procedures.
Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies
Qualifications
Current LVN license issued from the California Board of Vocational Nursing and Psychiatric Technicians
At least one year of nursing experience, preferably in home health, homecare or public health nursing.
Has an active CPR Card
Knowledge and compliance of professional nurse's act and standards of care
Must have a valid and active driver’s license
Must have car insurance
Liability insurance is optional
Must have an automobile in good working condition
Must have great interpersonal and communication skills
Must be proficient in the use of computers for computerized clinical documentation
