    Licensed Vocational Nurse (LVN) – Care Coordinator (Home Health) Position Summary:

    The LVN Care Coordinator plays a key role in managing patient care coordination for home health clients. This position ensures that all patient care activities are organized, documented, and delivered in compliance with clinical standards and regulatory requirements. The LVN Care Coordinator will work closely with clinicians, physicians, and office staff to facilitate smooth communication and workflow using Kinnser (WellSky) EMR.

    Key Responsibilities:

    Coordinate and schedule home health visits based on patient needs and clinician availability.

    Review clinical notes in Kinnser EMR to ensure accuracy, timeliness, and completeness.

    Follow up with field nurses and therapists for pending documentation, orders, or corrections.

    Communicate with physicians for verbal orders, plan of care updates, and clarifications.

    Ensure compliance with Medicare and agency policies in patient charting and care delivery.

    Monitor active patients for timely Start of Care (SOC), Recertifications, and Discharges.

    Assist in preparing reports and tracking patient status changes.

    Support the QA department in reviewing visit notes and ensuring documentation compliance.

    Handle patient and family inquiries with professionalism and empathy.

    Participate in case conferences, staff meetings, and care coordination discussions.

    Qualifications:

    Current Texas Licensed Vocational Nurse (LVN) license (or applicable state).

    Minimum 1 year of experience in Home Health (Medicare-certified preferred).

    Proficiency in Kinnser (WellSky) EMR required.

    Strong knowledge of OASIS, 485s, Plan of Care, and CMS compliance guidelines.

    Excellent organizational, communication, and time management skills.

    Compassionate, detail-oriented, and able to multitask in a fast-paced environment.

    Preferred Skills:

    Familiarity with PDGM structure and billing cycles.

    Prior experience coordinating with multidisciplinary clinical teams.

    Bilingual (English/Spanish) preferred but not required.
