📌 Key Responsibilities:

🩺 Skilled Nursing Visits – Wound care, medication teaching, vital checks, and patient assessments 📄 Documentation – Complete visit notes accurately in Axxess EMR (training provided) 📞 Communication – Report patient changes promptly to RN Case Manager/DON 👥 Patient Education – Teach patients and families about care plans and medications 🚗 Field Work – Reliable, timely visits to patients’ homes within San Antonio 📲 (Optional) Telehealth Coordination – Assist with simple remote monitoring follow-ups when needed

🔎 What We’re Looking For:

✅ Active LVN license in Texas ✅ At least 1 year of nursing experience (Home health preferred but NOT required) ✅ Good communication, professionalism, and reliability ✅ Comfortable working independently in the field ✅ Reliable transportation & auto insurance ✅ Bilingual (Spanish) a plus, not required
