About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated home health team in San Diego, CA. Responsibilities:
Provide direct patient care in home settings, including wound care, medication administration, and vital sign monitoring
Develop and follow individualized patient care plans in coordination with the supervising RN and care team
Observe, document, and report changes in patient condition to appropriate clinical staff
Administer prescribed medications, injections, and treatments per physician orders
Educate patients and family members on care procedures, medications, and health management
Maintain accurate and timely clinical documentation in compliance with state and agency standards
Coordinate with multidisciplinary care teams to ensure continuity and quality of patient care
Requirements:
Active Licensed Vocational Nurse (LVN) license in the state of California
Current CPR/BLS certification
Prior home health, skilled nursing, or clinical care experience preferred
Strong knowledge of wound care, medication management, and patient assessment
Excellent communication, time management, and organizational skills
Reliable transportation and valid California driver's license with proof of insurance
Compassionate, self-motivated, and able to work independently in a home care environment
