Licensed Vocational Nurse (LVN) – Home Health

Our agency’s mission is to provide professional and paraprofessional services to clients in their

homes assisting them to achieve the highest level of potential in their day-to-day self-care

activities.

Job Description:

• Provide direct patient care in a home setting, ensuring the comfort and well-being of

patients

• Administer medications and treatments as prescribed by the physician (Wound Vac, IV

Certified, Wound care, etc.)

• Monitor and record vital signs, such as blood pressure, temperature, and heart rate

• Implements plan of care initiated by the registered nurse / Referring doctor

• Educate patients and their families on proper healthcare procedures and disease

management

• Provides accurate and timely documentation consistent with the plan of care

• Participates in coordination of home health services, appropriately reporting the

identified needs for other disciplines (HHA, OT, PT, MSW, ST,) to the registered nurse

and/or Clinical Supervisor/Nursing Supervisor

• Participates in personal and professional growth and development.

-Uses equipment and supplies effectively and efficiently.

Requirements:

• Valid nursing license in the state of California

• Minimum 1 year of experience as an LVN in Home Health

• Proficient in taking and interpreting vital signs

• Full time / Part time position
