    Position: Licensed Vocational Nurse (LVN/LPN) – Private Duty/Field Nurse

    Reports to: Clinical Supervisor

    Compensation: $34 - $40/hour, depending on experience

    Schedule: Part-Time (16-24 hours/week)

    Location: Napa County

    About the Role

    We are seeking a Licensed Vocational Nurse (LVN) to provide continuous, one-to-one skilled nursing care in the patient’s home, typically through 8-hour shifts, under the supervision of a Registered Nurse. This role is ideal for an LVN who operates with great attention to detail and a big heart, providing high-quality, compassionate care while supporting the patient’s ongoing clinical needs.

    In this role, you will focus on direct patient care, implementing established plans of care, and following treatment protocols within the patient’s home environment. You serve as a vital link between the patient and the clinical team, helping ensure continuity of care and clinical excellence in accordance with:

    California Title 22 HHA regulations and CDPH standards.

    Accreditation Commission for Health Care (ACHC) standards.

    VA/TRICARE, Medi-Cal/CCS, and private insurance requirements.

    Key Responsibilities

    1. Clinical Excellence & Patient Care

    · Provide direct patient care as defined in the state Nurse Practice Act.

    · Implement the plan of care initiated by the Registered Nurse and make necessary revisions as patient status and needs change.

    · Deliver skilled nursing care, including medication administration and treatments as prescribed by the physician.

    · Use and prepare equipment and supplies effectively while adhering to aseptic techniques.

    · Provide patient and family/caregiver education regarding self-care techniques and information pertinent to the diagnosis and plan of care.

    1. Communication & Coordination

    · Prepare accurate and timely clinical notes consistent with the plan of care.

    · Identify and report condition changes or identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the Registered Nurse and/or Clinical Supervisor.

    · Communicate with community health-related persons to coordinate a holistic care plan.

    · Participate in coordination of home health services to support positive patient outcomes.

    1. Compliance & Administrative Standards

    · Ensure necessary equipment and supplies are available for patient care.

    · Participate in on-call duties as defined by agency policy.

    · Maintain strict adherence to CMS, ACHC, and Title 22 standards.

    · Participate in personal and professional growth and development.

    Position Qualifications

    Graduate of an accredited practical nurse or vocational nursing program.

    Minimum one (1) year nursing experience preferred for independent field assignment. Experience in home health, subacute, LTAC, or acute care preferred.

    Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.

    Possess and maintain valid CPR certification.

    Complies with accepted professional standards and practice.

    Competency matched to patient acuity.

    Demonstrates good verbal and written communication, and organization skills.

    Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
