  • Responsibilities

    Circle of Life Hospice & Palliative Care is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our hospice team. The LVN will provide quality nursing care to hospice patients in their homes, facilities, and other care settings while supporting patients and families through the hospice experience.

    Responsibilities

    Provide compassionate nursing care to hospice patients according to the plan of care.

    Perform patient assessments and monitor changes in condition.

    Administer medications and treatments as ordered.

    Provide wound care and other skilled nursing services as needed.

    Communicate changes in patient condition to the RN Case Manager and hospice physician.

    Assist with symptom management and maintaining patient comfort.

    Educate and support patients, caregivers, and families regarding the plan of care.

    Complete accurate and timely clinical documentation.

    Collaborate with the interdisciplinary hospice team.

    Follow all hospice, agency, HIPAA, and regulatory requirements.

    Respond appropriately to patient and caregiver needs and report concerns promptly.

    Maintain professional communication with patients, families, facilities, physicians, and members of the hospice team.

    Qualifications

    Current and unrestricted Texas LVN license.

    Current CPR certification.

    Previous hospice, home health, long-term care, or other clinical nursing experience preferred.

    Valid driver's license and reliable transportation.

    Current automobile insurance.

    Ability to work independently and manage multiple patient needs.

    Strong communication, organization, and documentation skills.
