Full-Time LVN – Magnolia Congregate Living Health Facility (6-bed facility) 💼 Full-Time | 12 hour shifts

Magnolia Congregate Living Health Facility is seeking a compassionate, skilled, and dependable Licensed Vocational Nurse (LVN) to join our growing team.

Our CLHF provides high-acuity, short-term and long-term care in a structured, patient-centered environment. We are looking for an LVN who is confident in clinical skills and passionate about quality care.

Responsibilities:

Provide direct skilled nursing care

Tracheostomy care and suctioning

G-tube feeding and medication administration

Wound care management

Medication administration and documentation

Monitor and report changes in condition

Collaborate with interdisciplinary team

Maintain compliance with CDPH regulations

Qualifications:

Current California LVN license (in good standing)

Experience with trach and G-tube preferred

Wound care experience a plus

Strong documentation skills

CPR/BLS certified

Compassionate and team-oriented
