We are seeking a motivated and compassionate Licensed Vocational Nurse (LVN) to join our home health and hospice team.

    Position Summary

    The LVN provides skilled nursing care to adult patients in their homes; coordinates care with the interdisciplinary team and serves as a primary point of contact under the Director of Nursing. This role requires strong clinical judgment, excellent communication skills, and a passion for patient advocacy.

    Responsibilities

    Provide skilled nursing care to adult home health and hospice patients

    Perform patient assessments and develop individualized care plans

    Coordinate care with physicians, families, and interdisciplinary team members

    Educate patients and caregivers on diagnoses, medications, and care plans

    Maintain accurate and timely clinical documentation

    Participate in on-call rotation and respond to PRN/emergency visits as needed

    Qualifications

    Current Texas LVN license (required)

    Minimum 1 year of home health or hospice experience preferred

    Current CPR certification (required)

    Valid Texas driver’s license, reliable transportation, and auto insurance

    Ability to communicate effectively in English

    Hepatitis consent/declination documentation

    Strong interpersonal and organizational skills

    Marketing experience or community outreach interest is a plus

    Physical & Work Requirements

    Ability to stand, walk, and lift patients as needed

    Comfortable working in varying home environments

    Potential exposure to blood-borne pathogens

    Flexible with local travel and scheduling
