Are you an LVN who wants to make a meaningful difference through one-on-one patient care? At ComForCare Home Care, our nurses provide compassionate, high-quality care that helps patients remain safe, comfortable, and supported in their homes.
We are looking for dependable, skilled, and patient-focused Licensed Vocational Nurses to join our growing home health team. This role is a great fit for LVNs who enjoy working independently while still being supported by an experienced care team.
What You’ll Do
Provide direct nursing care in patients’ homes according to the plan of care
Administer medications, injections, treatments, and wound care as ordered
Monitor vital signs, symptoms, and patient progress
Communicate changes in condition to the RN, physician, and care team as appropriate
Coordinate care with the interdisciplinary team
Complete accurate and timely documentation, including EMR documentation
Follow infection control, safety, HIPAA, and agency compliance standards
Support personal care needs within LVN scope of practice when appropriate
What We’re Looking For
Current California LVN license
Graduate of an accredited nursing program
Current CPR certification
Home health experience preferred
Wound care and venipuncture experience preferred
Comfortable with EMR documentation
Strong communication, observation, and time-management skills
Ability to work independently and coordinate with a care team
Valid California driver’s license, auto insurance, and reliable transportation
