    WHAT YOU WILL BE DOING:The Family Services Coordinator (FSC) is responsible for coordinating a wide range of support for potential organ and tissue donor families as well as creating a positive environment for the donation process within the hospitals. The FSC is responsible for maintaining an authorization rate as described within the Life Connection of Ohio (LCO) strategic plan by providing superior support to our families. This individual will work outside of normal business hours and must be able to flex their time according to activity.

    KEY RESPONSIBILITIESEssential Duty or Function1. Assumes responsibility for supporting the family throughout the donation process in a hospital setting.2. Responsible for the following during scheduled on-call days as assigned:

    A. Referral follow-up as directed.

    B. Chart evaluation to determine medical suitability

    C. Providing high quality bereavement support to potential donor families.

    D. Educating potential donor’s next-of-kin (NOK) about donation and completing appropriate paperwork for donation

    E. Completing the donor risk assessment interview (DRAI).

    F. Ensures effective communication with donor families, hospital personnel, physicians, related donation agencies and other LCO staff to promote a positive teamwork environment

    G. Completing accurate and timely documentation of donor related activities in accordance to established policies and procedures.

    1. Ensures strong collaboration with the recovery and hospital services departments to ensure the maximization of organ and tissue donation opportunities. 4. Collaborates with hospital services in the after action-review process to gather information for continuous improvement on the donor process. 5. Plays an active role in supporting continuous improvement within the team and organization.6. May perform other duties as required.

    JOB SPECIFICATIONS

    Education: Bachelor’s Degree is preferred or equivalent work experience in related field. Other professional certifications will be considered.

    Experience: Previous counseling, social work, work in crisis management or bereavement support experience is preferred.Skills and Abilities

    Must be able to work outside regular business hours.

    Requires valid driver’s license and access to an insured automobile or the ability to arrive and depart from work and work-related functions at expected times.

    This position requires on-call responsibilities and travel is essential to this position.

    Advanced level of sensitivity in providing emotional support to grieving families in the hospital setting

    Ability to assess family dynamics as the basis for a plan for support.Able to work well with all levels of personnel in hospital settings and within guidelines of each institution.

    Must be highly accountable and able to work autonomously. Must possess a high level of self-motivation and well-developed time management, organizational and problem-solving skills.

    Strong communication (verbal and written) and organizational skills.

    Basic computer and data entry skills, including Windows applications (e.g., Word, Excel & PowerPoint) required.

    Strong interpersonal skills required to develop close working relationships with hospital and health-care related personnel involved in organ donation and transplantation.

    Sensitivity and ability to ensure absolute confidentiality required.
