    Job Description

    JOB SUMMARY:

    The ideal candidate will have successful Implementation and Project Management experience with the ability to come up to speed quickly; building and managing successful relationships with both new and existing Customers, appropriately identifying their needs and goals in order to aid them in reaching success using our Population Health Management product.

    JOB RESPONSIBILITIES:

    • Overall project management of assigned Customers
      • From assignment/inception.
      • Managing the implementation of our Population Health software solution
      • Managing assigned Customer throughout their lifecycle
    • Create and maintain successful relationships with Customers to better understand and achieve their population health needs and goals
    • Work cross functionally within the company to communicate with all stakeholders in Customers’ success
    • Define and coordinate tasks and resources to ensure contract deliverables are on target to meet milestones, objectives, and goals
    • Ensure Customer expectations, timelines are set and met to the Customer’s satisfaction.
    • Develop and maintain detailed project plans, outlining tasks, timelines, and responsibilities to be monitored and tracked to ensure progress
    • Measure project performance using appropriate tools and techniques
    • Manage changes to the project schedule, project scope and/or project costs as needed/applicable
    • Perform risk management to minimize project risks
    • Track any/all open issues, working internally and with Customers to address and resolve
    • Actively work with team members to resolve open issues
    • Understanding and Dissemination of Validation for Claims and Clinical data as needed
    • Monitor and report on regular data transmission for errors or issues (did it get staged, get into UI) communicate to customer
    • Report and escalate issues as appropriate to management
    • Successfully manage the relationship with the Customer’s and all stakeholders
    • Establish and maintain relationships with third parties/vendors
    • Establish and maintain stakeholder governance communication and structure including monthly executive summaries
    • Establish and maintain Jira tickets, Project Schedules, Meeting Agendas and Minutes, Operational Reporting metrics
    • Consistently monitor and report on progress of the project to all stakeholders
    • Implement and manage project changes and interventions to achieve project timelines
    • Project evaluations and assessment of results
    • Present reports defining project progress, problems and solutions
    • Ensure that both Customer and company objectives, expectations and goals are being met

    QUALIFYING JOB KNOWLEDGE, SKILLS & ABILITIES:

    • Proven working experience in Implementation and Project Management within the Healthcare Industry
    • Knowledge of project management techniques and tools
    • Understanding/Experience with healthcare data
    • Payor/Provider Background preferred
    • Strong working knowledge of Claims and Clinical data
    • Understanding of interfaces
    • Knowledge of value based care, population health a plus
    • Experience building and managing project plans and schedules in Smartsheet is a plus
    • Accustomed to Customer-facing roles
    • Ability to speak to all levels within an organization, including C Suite
    • Outstanding communication skills
      • Written and verbal
    • Solid organizational skills, including attention to detail and multi-tasking skills
    • Strong working knowledge of Microsoft Office Suite
    • Ability to embrace change and company growth

    EDUCATION:

    • Required: BS Degree (or higher) or equivalent work experience
    • Preferred: MS degree in Computer Science; engineering or related subject
      • Preferred: PMP
