    FULL-TIME PHYSICAL THERAPIST (M-F)

    JOB SUMMARY

    Provides physical therapy services appropriate to patients’ needs, utilizing necessary resources to ensure optimal patient care in all areas of patient care at Weiss (acute care, inpatient rehab unit, outpatient therapy clinic, geriatric psych unit). Primary location in outpatient therapy clinic providing oversight and leadership to multidisciplinary clinical team in the clinic under direction of the director of rehabilitation and lead therapist/manager.

    JOB QUALIFICATIONS

    1. Graduation from an accredited physical therapy school with Bachelor’s, Master’s, or entry-level Doctorate degree or verification by the APTA of equivalent training in another country

    2. Current physical therapy license in State of Illinois

    3. Three years of relevant clinical experience required, with a minimum of two years progressive responsibility preferred

    4. Specialty certification strongly preferred; advanced clinical competency in relevant area of practice may be substituted

    5. BLS certification through the American Heart Association

    6. Demonstrated ability to lead and mentor staff

    7. Strong organization and prioritizing skills

    8. Basic English language competence for verbal and written communication in English language

    9. Effective interpersonal skills

    SUPERVISION

    Exercised: Oversees patient care delegated to rehab technician and/or PTA. Supervises physical therapy and/or PTA students. Orients, coaches, and mentors outpatient therapy team.

    Received: Reports to the Director, Rehab Program. May receive orientation, coaching and mentoring from department Lead Therapist.

    JOB RELATIONSHIPS (INTERNAL/EXTERNAL)

    Communicates effectively through frequent verbal and written interactions, including phone calls, with patients, visitors and/or department personnel and other departments, employees and/or medical staff. Establishes therapeutic relationship with patients / families, interdisciplinary team members and external contacts.

    Adjusts patient interaction and/or treatment to match each patient’s age-specific developmental, educational, or other related needs.

    JOB DUTIES (* denotes “essential function”)

    1. Demonstrates the WMH Customer Service Values, which are key in providing quality service to patients and customers. *

    2. Demonstrates advanced skill in evaluating, interpreting, planning, educating and implementing age-appropriate and functional physical therapy treatment, including but not limited to diagnostic procedures to assess nerve, muscle, joint integrity and to treatments utilizing modalities, therapeutic exercise, manual therapy, neuromuscular re-education, ADL training/use of adaptive equipment, community re-integration, visual-perceptual/sensory integration, splinting/orthotics, and/or wound care interventions.*

    3. Establishes and revises treatment plan based on physician orders, patient/caregiver input, evaluation results and patient progress.

    4. Serves as clinical resource for outpatient therapy team and/or physical therapy team.

    5. Ensures treatment outcomes meet program standards and reimbursement criteria and facilitate optimal discharge planning.

    6. Independently performs all duties required to maintain and complete patient records in a timely manner.

    7. Communicates effectively with patients, caregivers, other team members to coordinate treatment plans.

    8. Maintains confidentiality of patient records and information.

    9. May assist the Lead/Manager or Director in the coordination of department operations.

    10. Actively pursues professional development and educational opportunities to expand clinical expertise.

    11. Attends all mandatory and relevant in-services and meetings.

    12. Serves as clinical instructor for physical therapy students and interns.

    13. Promotes professional growth and program development/initiatives of outpatient therapy and/or physical therapy areas.

    14. Works to promote understanding of outpatient therapy and/or physical therapy roles within the rehabilitation program and the organization.

    15. Participates in and leads program development projects.

    16. Serves as a resource for other staff members, sharing professional knowledge/expertise with others.

    17. Manages patient caseload and daily operations efficiently.

    18. Assists in orientation process for outpatient therapy team, physical therapy team, volunteers, students, and professionals associated with the rehab program.

    19. Ensures appropriate delegation to and supervision of rehab technician, students, interns, and volunteers for outpatient therapy and/or physical therapy.

    20. Provides outpatient therapy team and/or physical therapy team with clinical direction and feedback to optimize patient care by coordinating in-services, meetings, caseload review, documentation review, and co-treatments as appropriate.

    21. Assists with communicating and supporting program initiatives.

    22. Identifies opportunities for improvement in department programs and/or processes and develops and/or participates in process improvement projects.

    23. Assists with defining and revising outpatient therapy and/or physical therapy policies and procedures.

    24. Provides in-services/training in specialty related topics for rehab team, other hospital departments, and community groups.

    25. Ensures safety of patients, caregivers, and co-workers by adherence to health and safety guidelines.

    26. Assists with other hospital system departments as needed and as identified by department leaders or hospital leaders.
