  • Responsibilities

    The Social Worker under the direct supervision of the Program Director is responsible for the provision of Love Joy ADHC’s social services including assessment, care plan development and advocacy for each participant. This position will participate in multidisciplinary team meetings and provide consultation to therapists, families, and participants.

    · Perform initial screening for Love Joy ADHC eligibility.

    · Orient visitors, family, and new participants to staff, program, and facilities. Assist with enrollment procedures.

    · Perform psychosocial assessments. Contact family and other past health providers as needed for histories. Obtain data related to financial situation and social supports that are needed for initial enrollment.

    · In conjunction with multidisciplinary team, develop individual treatment plans.

    · Perform social service interventions for participants and family. These measures may include supportive techniques, confrontation and realistic testing of situations and options, education about effects of illnesses on emotions and exploration of coping skills, crisis intervention and reality orientation.

    · Communicate regularly with family members, especially those in high-stress risk of placement situations.

    · Counsel families to involve them in carryover of treatment plan at home.

    · Provide information and referrals to participants and family.

    · Act as liaison for families and participants in dealing with public, private and social service agencies.

    · Consult with multidisciplinary team and develop discharge plans. Note in chart changes in these plans following staff conferences. Visit and collect information on discharge planning resources.

    · Do home visits prior to admission to assess living environment, transportation, and accessibility to facilities, family relationships, support, and functional abilities.

    · Make annual home visit when and if circumstances of participant changes from initial home visit.

    · Document appropriate participant and family contacts.

    · Prepare quarterly progress notes and other reports as necessary, date and sign. All services must be documented and signed on the day the services are provided.

    · Conduct participant's reassessments every six months.

    · Develop and lead appropriate clinical and family support groups.

    · Provide staff support for Advisory Board and Utilization Review Committee.

    · Provide outreach to recruit new participants.

    · Organize and maintain referral and waiting list system.

    · Assist in the development of in-service training for staff.

    · Supervise Aides as assigned.

    · Other duties and projects as assigned.

    Position Requirements:

    · Master’s degree in social work

    · Strong written and verbal communication skills.

    · Have and maintain current First Aid and cardiopulmonary resuscitation (CPR) certification within the first six months of employment.

    · Present pre-employment physician's health clearance including a negative TB skin test and/or chest X-Ray.

    · Ability to conform to the applicable laws, rules, and regulations.

    · Be of good character and reputation of personal integrity.
