Love Joy is an Adult Day Health Care located in Rancho Cordova, CA. Our participants are adults ages 18 and up with various cognitive, physical, or psychological challenges. We provide Maintenance Nursing care, Occupational/Physical/Speech therapy, Activities, and Social Services including group education, one on one mentoring/counseling. Program hours are Monday - Friday, 8:00-2:00.

We are seeking a part-time or full-time RN to join us as we grow our program. The Registered Nurse is responsible for the provision of nursing services to persons receiving medical care, including assessment, care plan development and monitoring of each participant. This position will participate in interdisciplinary team meetings and provide consultation to therapists, families, and participants.

The ideal candidate has exceptional organization and time management skills, energetic, excellent clinical reasoning, great communicator and problem solver, compassionate and desire to participate in Patient First care.

Job Duties Include:

· Initial assessments and reassessments, and quarterly assessments

· Assess each participant to determine needs for personal care and assistance in activities of daily living such as bathing, grooming, toileting and eating. Include these needs in the plan of care and supervise the provision of these services.

· Proficient use of Electronic Medical Record

· Provide active role as member of the multidisciplinary team to assess the nursing needs of the participant.

· Develop the nursing component in the plan of care for each participant capable of benefiting from a continence-training program.

· Document all skilled nursing care provided on the day the service is given

· Provide health education and counseling to the participant and family when indicated by the plan of care

· Aid the participant in obtaining medical services from providers other than adult day health staff

· Monitor, administer and record prescribed medication

· Provide liaison to the participant's personal physician, caregiver, and other members of the participant's healthcare team

· Provide supportive and restorative nursing care as indicated by the treatment plan

· Provide observation and monitoring of health status

· Other duties and projects as assigned.
