  • Responsibilities

    The Occupational Therapist provides an occupational therapy program for the maintenance and rehabilitation of frail elderly participants.

    · Initially assess all participants and reassess every six months those participants to whom providing or supervising services.

    · Develop individual and group occupational therapy program as required by the individualized plan of care. The occupational therapy program shall meet the following goals:

    a. Decrease or eliminate disability during participant's initial phase of recovery following injury or illness.

    b. Increase or maintain the participant's capability for independence.

    c. Enhance the participant's physical, emotional, and social well-being.

    d. Develop function to a maximum level.

    e. Guide participants in the use of therapeutic, creative and self-care activities for improving function. These may be therapeutic exercises, sensory stimulation and coordinated exercises concentrating on the range of motion of the upper extremities.

    · Record treatments, each day provided to participants in their health record. Each entry shall be signed and dated.

    · Record quarterly progress notes. Each entry shall be signed and dated.

    · Define the responsibilities of the occupational therapist and the duties assigned to the auxiliary personnel by the individual treatment plan.

    · Maintain equipment and supplies for creative skills. This may include, but are not limited to leatherwork, weaving, needlework, ceramics, woodworking, painting, and graphic arts.

    · Maintain supplies for adapting equipment for reeducation in activities of daily living.

    · Evaluate the need for splints or slings to increase or maintain functional use of the upper extremities.

    · Act as a member of the assessment team in the evaluation of the participant's rehabilitation potential.

    · Review and complete all discharge summaries on a monthly basis.
