    Job Summary: Our team is seeking an enthusiastic Licensed Practical Nurse (LPN) to assist in maintaining a positive physical, social, and psychological environment in accordance with the resident’s needs and facility philosophy. This is a part-time, night shift opportunity. The LPN is responsible to the Resident Care Coordinator for the planning, directing, supervision and delivery of direct resident care; for maintaining accurate and complete medical records; for utilizing safety measures in provision of care; for preventing spread of disease and infection by following established universal precautions; for attending staff meetings, classes and on-going in-service programs to improve knowledge and skills.

    Job Duties:

    Actively supports Lutheran Haven’s Mission and Ministry.

    Demonstrates a willingness to be an effective team member by fostering positive relationships with teams and residents.

    Provide consistent leadership to mentor and motivate staff towards total quality resident care.

    Visit with each individual resident two (2) times per week to address any needs they may have and maintain communication with Resident Care Coordinator on resident requests to be addressed by Resident Care Coordinator.

    Provides care according to physicians’ orders and in conformance with organized nursing practice, established standards, and the administrative policies.

    Supervises daily functions of the ALF, from employee supervision to delivery of resident care.

    Orders prescribed medications, supplies and equipment as necessary, in accordance with established policies.

    Requisitions and arranges for diagnostic and therapeutic services as ordered by the resident’s physician.

    Maintains documentation which accurately and completely records data pertaining to resident’s care, condition and treatment in accordance with HIPAA.

    Consults with the Resident Care Coordinator, as necessary, to provide input regarding resident care needs, staffing requirements, and supply needs for the unit.

    Make rounds with medical personnel as requested, assists in examination and/or treatments of residents and carries out orders.

    Other duties as assigned.

    Minimum Qualifications:

    Licensed Practical Nurse currently licensed to work in the state of Florida

    Minimum of one (1) year verifiable, successful clinical experience; experience in geriatrics or rehabilitation environment preferred.

    Current CPR card.

    Basic computer skills.

    Ability to demonstrate sound judgment and ability to function on a professional level with clients, staff, and other outside professionals during stressful or emotional situations.
