    GENERAL SUMMARY:

     

    Under the supervision of the RN Supervisor of the Transitional Care Unit, the Licensed Practical Nurse administers direct nursing care to patients and performs a variety of indirect patient care duties. The License Practical Nurse is responsible for all professional working policies pertaining to this section.

    EDUCATION:

     

    Graduate of an accredited Licensed Practical Nurse Program required.

    A pharmacology course of at least forty-five hours must be included in the basic education curriculum or have been attended post graduation.

     

    WORK EXPERIENCE:

     

    Minimum 2 years experience in a long term care setting preferred.

    SPECIAL EMPLOYMENT REQUIREMENTS:

     

    Possession of a current license as a Practical Nurse issued by the Pennsylvania State Board of Nurse Examiners required.

    Must maintain the standards of nursing care and implement the policies and procedures of the Nursing Department and Allied Services.

    Must be able to demonstrate the knowledge and skills necessary to provide individualized care appropriate to the age specific need and culture of the patient served.

    Performs all functions of a licensed practical nurse within the scope of practice as outlined by the regulatory agencies such as such as the PA State Board of Nursing and the Pa. Department of Health. Delivers nursing care according to the standards of care as outlined by the various associations such as The Association of Rehabilitation Nurses, Pennsylvania Nurses Association, and American Nurses Association Standards of Clinical Practice

    Assists with rehabilitation of patients and delivers care in accordance with the plan of care, answers call lights; assists patients with personal hygiene and other activities of daily living; and records and reports all observed symptoms, reactions, and changes in the patient's condition to the Registered Nurse.  Participates in unit rounds regularly. 

    Administers medications and performs treatments, in accordance with policy and procedure, to assigned patients and/or all patients assigned to a team. Documents medications and treatment. NOTE: LICENSED PRACTICAL NURSES APPROVED FOR ADMINISTRATION OF MEDICATIONS MUST HAVE SATISFACTORILY COMPLETED A MEDICATION COURSE. 

    Under the supervision of a Registered Nurse, monitors patient’s physical, psychological, social and rehabilitative needs by collecting pertinent data using appropriate evaluation techniques.  Assists in admitting, discharging, and transferring patients. 

    Reports pertinent observations and reactions and changes in patients condition to the appropriate person (i.e., RN, physicians, Unit Manager, Senior Staff RN, Nursing Supervisor, or designee) and records those observations accurately and concisely. 

    Assists with discharge planning as related to the nursing needs of the patient. Assists with completion of the discharge instruction sheet.

    Transcribes and carries out physician orders. Assists physicians with special tests and procedures. 

    Provides patient education as determined by the nursing care plan. Establishes and maintains communication and utilizes teaching opportunities with patients, and other significant individuals. 

    Responsible for evaluating and documenting patient’s functional abilities and assigning accurate Fim score as related to the nursing department. 

    Assists in monitoring environment for potential safety risks to patient, visitor, and staff. Assists in reporting, evaluation, intervention, investigating, documenting, and assuring follow up on patient safety issues. 

    Participates in team conferences, development of nursing care plans, and team shift reports. Maintains and promotes positive relationships with patients, families, and other personnel. 

    Guides and directs nurses aides in rendering patient care. 

    Responsible for data entry re: FIM/PPS, LOA and discharges as related to the nursing department.  

    Maintains patient confidentiality, follows HIPPA regulations, assumes role of patient advocate, seeks available resources to help formulate ethical decisions.

    SKILLS, ABILITIES & PHYSICAL DEMANDS:

    • Excellent verbal and written communication, and listening skills.
    • Basic reading and comprehension skills.
    • Basic numeric reasoning skills.
    • Ability to bend, stoop, reach, stand, push, pull and lift cartons weighing approximately 50-100+lbs repetitively.

    REQUIREMENTS

    • Maintain an acceptable Motor Vehicle Record
    • Obtain and maintain a DOT physical card
    • Must be able and willing to fill in for driver as needed

    WORK SCHEDULE:

    Tuesday through Saturday;  6:00am until assigned deliveries are complete.
