Job Description:

Bella of Texas is currently seeking a full-time Registered Nurse (RN) to join our home health line of service. The ideal candidate will possess a passion for providing exceptional care to patients in the comfort of their homes.

Responsibilities:

• Conduct home care visits in select senior care communities in the DFW area for various purposes including SN initial assessments, OASIS admissions, re-cert visits, Sup & SN visits.
• Ensure accurate and thorough documentation utilizing home health forms, OASIS charting, and adhering to Medicare guidelines, rules, and regulations to ensure compliance with all State regulations.
• Demonstrate proficiency with electronic charting systems, particularly Axxess, to maintain accurate and up-to-date patient records.
• Collaborate effectively with interdisciplinary teams to develop and implement comprehensive care plans tailored to each patient's individual needs.
• Provide patient and family education regarding healthcare management, disease prevention, and treatment options.

Requirements:

• Valid Registered Nurse (RN) license in the state of Texas.
• Proficient knowledge and experience with home health forms, OASIS charting, Medicare guidelines, rules, and regulations.
• Familiarity with electronic charting systems, particularly Axxess, is preferred.
• Strong interpersonal and communication skills with the ability to work effectively in a multidisciplinary team environment.
• Compassionate and patient-centered approach to care delivery.
• Ability to adapt to changing environments and prioritize tasks effectively.
• Valid driver's license and reliable transportation for travel within the DFW area.
