    💼 Your Role

    As a Licensed Medical Social Worker, you will be an essential advocate for our patients and their families — providing emotional support, coordinating care, and ensuring that every individual receives the highest standard of service and compassion.

    🩺 What You’ll Do

    Provide professional medical social work services in alignment with hospice standards of care.

    Conduct psychosocial assessments to understand each patient’s emotional and social needs.

    Collaborate closely with physicians, nurses, and families to create individualized care plans.

    Make follow-up visits to evaluate progress and adjust care strategies as needed.

    Develop interventions tailored to patients’ evolving needs and circumstances.

    Maintain accurate, timely, and confidential documentation.

    🎓 What You Bring

    Education: Master’s degree in Social Work (MSW)

    Licensure: Michigan LMSW or LLMSW accepted

    Experience: Minimum one year in healthcare, hospice, or related setting

    Certifications: Current CPR certification and valid driver’s license

    Bonus: Psychiatric or behavioral health experience preferred
