    Job Description

    GENERAL DESCRIPTION:

    The key purpose of this position is to assist clients obtain and maintain the best possible housing solution for their individual circumstances. The Housing Case Manager conducts assessments, determines eligibility, develops individualized service plans, links individuals/families with community based services and resources, manages financial assistance funds (security deposits, rents, etc.), and provides case management services in conjunction with community partners. The Case Manager is responsible for ensuring that clients receive sufficient and appropriate care and that the mechanisms to end homelessness are being accessed effectively. The Case Manager will also utilize the HMIS system and work with the team to ensure program compliance.

    ESSENTIAL RESPONSIBILITIES:

    • Represents MUST Ministries in the most positive manner with clients, suppliers, and the community we serve. Interacts effectively with a diverse group of faculty, staff, and clients of our service, learns and uses operating practices of MUST Ministries.
    • Upholds the Mission Statement: Serving our neighbors in need … transforming lives and communities in response to Christ's call.
    • Handles all information with tact and discretion and recognizes the confidential nature of MUST Ministries.
    • Supports learning through performance of essential job functions and performing other duties and functions as necessary or as assigned.
    • Participates in staff meetings, in-service training, and workshops deemed appropriate by the Sr. Director, Program Services or the VP, Housing.
    • Screens all applicants and determines eligibly of applicants; makes appropriate referrals as needed.
    • Orients all clients on the program policies and procedures.
    • Provides Case management for individual clients including, conducting needs assessments, creating individualized service plans to achieve self-sufficiency.
    • Serves as the program representative at team meetings presenting the issues and concerns of the clients.
    • Increase client’s awareness of available community and agency resources and assist them in accessing these services.
    • Coordinates the provision of substance abuse, employment, and mental health services with the appropriate agency/program.
    • Develops and maintains partnerships and networks with community agencies that best serve the general specific needs of homeless clients.
    • Maintains all client charts and records according to established standards.
    • Completes data collection and reporting requirements for various funding sources.
    • Performs other duties as assigned.

    ESSENTIAL JOB REQUIREMENTS:

    • Bachelor’s degree in Social Work, Human Services or related field and 2+ years of experience working with mental health, disabled, veterans and/or diverse populations; MSW preferred.
    • Certified Addictions Counselor (CAC) or Certified Peer Specialist preferred.
    • Proficiency in Microsoft applications including Word, Excel and Outlook, HMIS reporting systems;
    • Strong organizational, verbal and written communication skills.
    • Ability to establish and maintain relationships with community partners.
    • Excellent customer service skills.
    • Skilled in documentation.
    • Ability to work in high intensity and emotional atmospheres where clients seek assistance, conflict resolution and verbal de-escalation training preferred.
    • Familiarity with social service and state agency provided assistance programs.
    • CPR and First-Aid certified a plus.
    • Personal qualities of integrity, credibility, and a commitment to and passion for MUST’s mission.
    • Must have a valid Driver’s license and current insurance.

     

    PHYSICAL AND WORK REQUIREMENTS:

    • Work is performed both in the field and in an office environment.
    • May be required to lift up to 20 lbs.
    • Requires sitting, standing, sufficient mobility and physical reflexes.
