    Carefield Living builds and operates outstanding assisted living/memory care communities by focusing on the needs and desires of our community members, their families and friends, our Carefield staff, and the community at large. Our goal is to help our community members grow and strengthen their network of support and friendship, gaining a greater sense of purpose and belonging.

    Carefield Living has collaborated with leading experts in the fields of cognitive impairment and memory loss to create our Lightfinder Memory Care Program, a proprietary program that addresses key risk factors associated with the development and progression of memory loss. As an assistant to the Memory Care Director at a Carefield community, you will have the opportunity to coordinate and implement the components of this program and have a direct impact on the lives of our residents and their families.

    JOB SUMMARY:

    This position supports a well-rounded and stimulating program of activities for seniors that fosters a positive atmosphere and meets the spiritual, cultural, physical, intellectual, and social needs of residents both inside and outside the Community.

    This position is also responsible for assisting the Memory Care Director in day-to-day operations of the Memory Care Community. Expectations are to coordinate and implement programs and services designed for residents with cognitive impairment; meet or exceed standards of care; encourage and assist each Memory Care resident to function at their highest level of ability; and by word and action reassure families and friends that residents are safe, secure, and well cared for.

    JOB DUTIES:

    ESSENTIAL FUNCTIONS

    • Support the mission, vision, and culture of the organization through positive communication and leadership.
    • Contribute to Community relationships by demonstrating cooperation and professional conduct with residents, families, fellow associates, physicians, pharmacies and other vendors or persons.
    • Contribute to team effort by being flexible in work assignments; by furnishing support; by taking initiative; and by understanding how this position affects and compliments all other Community positions.
    • Able to maintain work pace appropriate to given work load.
    •  Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance.
    • Assist in supervision of Memory Care staff and assist the Memory Care Director in training of other staff members.
    • Assist in the implementation of the Memory Care program mission, goals, and philosophy.
    • Maintain stimulating activity programs that challenge residents mentally, physically, and spiritually and that match individual preferences and capabilities.
    • Maintain environments that encourage memories, promote feelings of well-being, and stimulate the senses in both personal and communal areas of the community.
    • Assist residents with activities of daily living such as bathing, dressing, grooming, dining, continence care, mobility, and other functions as designated in the residents’ service plan.
    • Respond to all resident concerns and requests (including call lights) in an appropriate, timely, and courteous manner.
    • Assist residents at meals by helping with menu selection as necessary, resetting tables, serving meals, delivering meals to rooms, and laundering of table linens on designated shifts.
    • Lift and/or transfer residents as needed.
    • Take and record temperature, pulse, respiration, monthly weights, and blood pressure as assigned.
    • Make resident rounds every two hours, as required in residents’ service plans, or as circumstances dictate.
    • Participate in the new resident welcome process.
    • Consistently contribute to the quarterly newsletter.
    • Recognize changes in residents’ conditions during daily interaction and report such changes to the Resident Care/Services Director.
    • Use approved charting criteria and procedures to record pertinent information in resident charts.
    • Ensure cleanliness of residents’ rooms and all other areas of the Community.
    • Appropriately assist residents and resolve concerns without having to be asked.
    • Report incidents or accidents involving residents to the Memory Care Director and Resident Care/Services Director and record all necessary information on the Resident Incident and Accident Reporting form.
    • Maintain a well rounded and stimulating holistic activity program, which addresses the needs and limitations of residents.
    • Assess the needs of the residents. Observes when a resident’s needs have changed and create an activity plan to encompass the changes.
    • Maintain interior space of the Community so that the resident can wander freely and safely.
    • Implement Life Enrichment portion of Memory Care Program when working with memory care residents.
    • Decorate for holidays, special occasions and assure community is always appropriately decorated.
    • Lead large group activities, e.g. discussion groups, sing-a-longs, exercise classes.
    • Arrange for appropriate entertainment for residents.
    • Help instruct and assist residents with gardening, arts, and craft projects.
    • Drive Community van and assist residents with entering and exiting van.
    • Schedule doctors’ appointments and ensure transportation to and from the facility.

    JOB SPECIFICATIONS:

    SKILLS

    • Experience in caring for seniors in an assisted living or skilled nursing environment.
    • Completion of training program for certification in states mandating such.
    • Valid First Aid and CPR Certification.

    EFFORT

    • Maintain mental alertness, attention, and concentration for necessary periods.
    • Apply common sense understanding and carry out instructions (written, oral or diagrammatic).
    • Adapt to situations requiring the precise attainment of set limits, tolerances, or standards.
    • Regularly required to sit and talk or listen.
    • Able to stand, stoop and bend frequently, lift up to 20 pounds and push up to 40 pounds. Also must have the ability to talk, hear, sit, use hands and fingers, handle/feel objects, tools or controls, and reach with hands and arms.
    • Specific vision requirements for this job include close vision, depth perception, and the ability to adjust the focus of the eyes.
    • Possess adequate physical stamina to move freely about the Community and assist residents where needed, including in emergency situations.

    WORKING CONDITIONS

    Must be willing and able to work required and scheduled shifts, including evenings, weekends, overtime, on-call and holidays.
