    WHAT YOU WILL BE DOING:

    The Care Coordinator is a key member of the Intensive Outpatient Care Program (IOCP) care team. The IOCP is a care support program for patients with complex health conditions.  We use a team-based approach to support patients based on their health goals.  We aim to improve their health, build hope that health improvement is possible, improve their self-management skills.  This can improve health outcomes and reduce costs and inappropriate utilization. The Care Coordinator works in collaboration and continuous partnership with “high-risk” patients with chronic health conditions and their family/caregivers(s) to help navigate care from clinic/hospital/specialty providers and staff, and community-based organizations.

    The Care Coordinator works closely with a Nurse Manager, who leads the IOCP Care team, and with the participants' primary care provider (PCP) and primary care clinic staff members. The Care Coordinator is the primary IOCP point of contact for the patient and works under the supervision of the Nurse Manager to support the patient in achieving their goals.

    • Serve as the primary contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
    • Find out the patient's goals of care and develop an achievable Action Plan with the patient, and family/caregiver(s)
    • With Nurse Manager work with patients to plan and monitor care
    • Assess the patient's unmet health and social needs and assist the patient in accessing available services
    • Monitor patient progress and facilitate changes as needed, including updating the patient's Action Plan
    • Create ongoing processes for patient and family/caregiver(s) to request support
    • Deliver person-centered care coordination, including working with a patient in a way that follows their needs and preferences, increasing patients' ability for self-management and shared decision-making and increasing health literacy through culturally and linguistically appropriate education
    • Identify and link patients with community resources to facilitate referrals and respond to social service needs
    • Track and support patients when they obtain services outside the practice such as Emergency Departments, Hospitals, and other health care facilities.
    • Follow-up with patients within 24 hours of an emergency room visit or hospital discharge
    • Provide relevant self-management support for patients with chronic illnesses as identified by clinical teams
    • Work with patients both in person and over the phone to remind and review their plan of care and progress towards their goals
    • Support the patient in managing referrals, as needed, to appropriate agencies required to assist the client in achieving the goals and objectives identified in their Care Plan
    • Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns.
    • Assist patients in problem-solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance)
    • Identify and utilize cultural and community resources.
    • Establish and maintain relationships with identified service providers
    • Assist with data collection and generation of patient summary reports 

    • Participate in Performance Improvement/Continuous Quality Improvement activities, as assigned
    • Attend all IOCP training courses/webinars and meetings

    SPECIAL SKILL REQUIREMENTS

    • Demonstrate excellence in both internal and external customer service 

    • Understand and effectively communicate HIPAA compliance, corporate compliance, and 
client confidentiality 

    • Be a good listener and empathic supporter of patients and their caregivers

    WHAT YOU NEED:

    • LPN or MA or Health Coach with a minimum of 36 months of experience in a health care setting. Preferred: Associate's or Diploma's degree in a health-related field

    • Strong written and verbal communication skills 

    • Teamwork orientation 

    • Able to take and follow through with delegated tasks and accountability 

    • Resourcefulness in problem-solving 

    • Proven data analysis skills 

    • Proven organizational skills 

    • Beginner to intermediate proficiency in Microsoft applications: Excel, Word, Outlook, 
PowerPoint 

    • Ability to demonstrate excellent customer service and strong cultural competency 

    • Ability to adhere to strict confidentiality guidelines

    PREFERRED QUALIFICATIONS

    • Two (2) years' experience working with complex patients with multiple chronic conditions and social and behavioral needs
    • Prior experience with care coordination/care management in a medical setting 

    • Prior experience with managing competing priorities 

    • Medical terminology 

    • Bi-lingual skills 

    • Experienced with public speaking, group training 

    • Experience in Project Coordination
