We are on a mission to improve healthcare for underserved patient communities. Community health centers and safety-net primary care clinics are the only sources of healthcare for millions of patients who struggle with both social and clinical needs. Yes, these clinics are overwhelmed, understaffed, and under-resourced to meet the needs of their Medicaid and Medicare patients; physicians are burning out with more than 50 appointments per day and less than 7 minutes per visit. It’s no surprise that unresolved systemic health inequality results in worse care quality.

Our company is the first tech-enabled care team that empowers safety-net primary care systems and the Medicaid & Medicare patients they serve. We act as an extension of the clinical staff to provide a personalized, high-touch care experience while addressing patients' barriers to care such as lack of transportation, housing/food insecurity, ad mobile phone access. Internally, we are building a unique care delivery platform to ensure clinical best practices, integrate care with community-based organizations, and automate administrative work so that we can focus on time with our patients.

Here’s how you'll make a difference:

• Provide best-in-class virtual preventive care for underserved patients using our internal care delivery technology and your excellent clinical judgment

• Develop and refine clinical programs to support our patients holistically and address barriers/gaps in their care

• Work with our product team and provide feedback to improve our platform for our growing care team

We’d love to hear from you if you have the following:

• 5+ years of clinical experience in complex care, chronic care, and primary care

• Technology-savvy and experienced in digital-first healthcare (e.g. telemedicine), or an appetite for learning it now

• Passionate serving patients with complex social and healthcare needs and building a more equitable healthcare system

• Excitement to work in an energetic and dynamic team environment with a bias towards action to meet the needs of our communities

• Board certification or eligibility in Family Medicine or Internal Medicine

• Bi-lingual, Spanish and English speaking

• Ability to obtain a license in any state

• Must have DEA license
