JOB SUMMARY
The Social Worker helps patients understand and cope with their condition(s). S/he provides direction for routine aspects of non-medical problems of patients and their families. A major focus is related to planning and post-discharge of patients with outside agencies, nursing staff, and relatives. This position includes providing community resources, planning transport, sharing resources for food and housing, providing health insurance coverage information, planning home care services, supporting appropriate referrals, addressing economic stability, providing education, and supporting patients in the social and community context.
WHAT YOU'LL DO:
- Work closely with the Population Health nurse to coordinate team-based care to provide health services to individuals, families and/or their communities through effective partnerships with patients, their caregivers, and their physician.
- Interviews, coordinates, and plans programs to meet the social and emotional needs of patients and patients' families in a hospital setting.
- Assists families in crisis situations in understanding the implications and complexities of the medical situation and its impact on lifestyle.
- Conducts financial assessment and referral services.
- Arranges for in-home care and equipment.
- Provides advance directive assistance.
- Educates families on treatment options.

QUALIFICATIONS:
- Bachelor's degree in Social Work required, Master's degree in Social Work preferred.
- Three years' experience in an acute care or healthcare organization setting preferred.
JOB INFORMATION:
- Starting wage $29.60 to $31.69 based on qualifications.
- Full-time, day shifts.
