HCT seeks an experienced nursing professional to serve as the Manager of Case Management for a 350+ bed hospital in California. Responsibilities include, but are not limited to:

• Provide hands-on leadership for a hospital case management department.
• Ensure case management policies and processes are consistently followed, educate staff on effective case management practices using GMLOS guidelines, improve multidisciplinary rounding functions, strengthen staff accountability, and support departmental operational excellence.
• Establish outstanding rapport and coordination with team, staff, and physicians. 

Requirements:

• Education: BSN degree required; Master's degree in related field preferred; RN required.
• Experience: Minimum 3 years progressive case management leadership experience in an acute care hospital. Experience managing registered nurses, social workers, and discharge planners is strongly preferred. 
• Licensure: Current CA RN license required.
• Certifications: BLS through AHA required or completed during onboarding. 
• Computer Skills: Meditech.
