Responsibilities
- Monday – Friday (40 hours)
- Lead and manage intensive support services including supervised living, staffed living, in-home supports, and ACT 248 programs.
- Promote and uphold Community Care Network (CCN) values across the Intensive Supports Team.
- Collaborate with internal teams and external agencies to address individual needs effectively.
- Develop staff schedules and ensure appropriate coverage to maintain service continuity and meet productivity targets.
- Train staff on implementing person-centered plans to enhance independent living skills.
- Ensure timely, accurate documentation aligned with DAIL, CCN, and Medicaid requirements.
- Assist teams in identifying cost-effective, independent housing options in the least restrictive settings.
- Build relationships with local landlords to expand affordable housing options in collaboration with the Residential Developer.
- Maintain personal familiarity with individuals supported to ensure continuity of care.
- Ensure compliance with all state, federal, and accreditation guidelines, including the State System of Care Plan.
- Foster partnerships with families, guardians, community members, and state agencies for holistic support.
- Contribute to quality assurance and continuous improvement efforts in service delivery.
- Manage program budget to ensure financial sustainability.
Requirements
- Bachelor’s degree in a related field preferred; equivalent experience accepted
- Minimum 4 years in developmental disabilities services; intensive supports experience preferred
- Qualified Developmental Disabilities Professional (QDDP) certification preferred
