    Salary and Benefits

    • The pay range for this position is based on experience ($65,000 - $70,000)

    Responsibilities

    • Monday – Friday (40 hours)
    • Lead and manage intensive support services including supervised living, staffed living, in-home supports, and ACT 248 programs.
    • Promote and uphold Community Care Network (CCN) values across the Intensive Supports Team.
    • Collaborate with internal teams and external agencies to address individual needs effectively.
    • Develop staff schedules and ensure appropriate coverage to maintain service continuity and meet productivity targets.
    • Train staff on implementing person-centered plans to enhance independent living skills.
    • Ensure timely, accurate documentation aligned with DAIL, CCN, and Medicaid requirements.
    • Assist teams in identifying cost-effective, independent housing options in the least restrictive settings.
    • Build relationships with local landlords to expand affordable housing options in collaboration with the Residential Developer.
    • Maintain personal familiarity with individuals supported to ensure continuity of care.
    • Ensure compliance with all state, federal, and accreditation guidelines, including the State System of Care Plan.
    • Foster partnerships with families, guardians, community members, and state agencies for holistic support.
    • Contribute to quality assurance and continuous improvement efforts in service delivery.
    • Manage program budget to ensure financial sustainability.

    Requirements

    • Bachelor’s degree in a related field preferred; equivalent experience accepted
    • Minimum 4 years in developmental disabilities services; intensive supports experience preferred
    • Qualified Developmental Disabilities Professional (QDDP) certification preferred
