Maple Star Colorado is a multi-service agency providing Foster Care, Clinical and Community Services. Maple Star began providing foster care in Denver in 1994 and has since grown to provide in-home, community and office based services throughout the Front Range. The Foster Care Placement Coordinator is responsible for placement of foster youth into Maple Star Foster Homes. This position also includes retention efforts for Maple Star placements and homes.
Essential Duties/Responsibilities
Serve as liaison to counties, i.e. communicate acceptance or deferral reasons for foster youth placement referrals as immediately as possible, inquire further about referrals where information is unclear or missing to help in the matching process, and respond to additional placement questions from referral sources as needed.
Coordinate appropriate placements of youth in Maple Star Foster Homes and based on an assessment of the history of the child referred, that best meet the child’s needs and placement criteria.
Determine if placement will be receiving any other Maple Star services and communicate with internal program staff in order to facilitate admission to programs as soon as information is available.
Coordinate the intake process for children being placed in the program, i.e. completing the Maple Star specific placement referral, discussing the placement with appropriate foster home matches, scheduling the intake, distributing the intake paperwork, requesting & tracking required documentation from the county, caseworker, guardian, child, and foster parents as needed.
Request, secure, & collect special circumstances placement letters and clothing allowances from required parties as needed prior to placement.
Coordinate and evaluate any pre-placement activities/”ice breakers”, and/or transitional visits when applicable.
Develop and maintain relationships with counties in order to increase Maple Star as a preferred placement agency and facilitate better matching of foster youth in Maple Star Foster homes.
Presence and participation in relevant County & CPA meetings across all regions served by Maple Star
Creating initial foster parent, foster child, and new home resource binders.
Education
Required: Bachelor’s degree in Social Service or related field; or
Preferred: Master’s degree in a Social Service or related field
Experience
2 years of relevant experience in addition to degree
Experience within Foster Care system & knowledge of Vol 7 Colorado State Rules & Regulations
Supervisory experience preferred
Prior placement supervisory experience in a foster care setting preferred.
Certificates, Licenses, Registrations
Proof of valid Driver License with clean driving record, as well as valid auto insurance
Ability to pass fingerprinting and background checks
