About your Role:
The Foster Home Supervisor is responsible for monitoring and providing on-going consultation to foster parents. Home Supervisors also are the liaison between departments of social services and foster parents. They monitor each youth in placement, develop case plans, and provide case management services.
Direct supervision of Foster Home including but not limited to:
Provide on-going monitoring and consultation to foster homes with an emphasis on safety.
Meet with the foster parents face to face a minimum of two times per month
Prepare and monitor an annual training plan with the foster parents
Complete annual SAFE Home Update evaluations for each foster home on caseload according to SAFE standards
To advocate for the foster parents, while keeping the child’s best interests as a priority.
Support families and work through crisis with the families to prevent placement disruptions while completing the necessary documentation, particularly Critical Incident reports within 24 hours of the incident.
Assist with the coordination of agency resources & and arrange for support services within the community
Communicate with foster care providers about upcoming events and assist with Support Group meetings and social activities such as holiday parties, parents’ night out and picnics
Direct case management of youth placed in Foster Home including but not limited to:
Provide on-going monitoring and consultation to youth placed in foster homes.
Responsible for maintaining an average caseload of 15-25 youth in care.
Meet with the youth in care face to face a minimum of 1x per month
Schedule and conduct service team meetings quarterly
Does the Following Apply to You?
Bachelor’s degree in Human Service, Social Work or related field required
Minimum of two years’ experience in youth service programming, including experience in the field of foster care and child welfare, one year experience working in a department of social services preferred
Experience within Foster Care system & knowledge of Vol 7 Colorado State Rules & Regulations preferred
Proof of valid Driver License with clean driving record, as well as valid auto insurance
Ability to pass fingerprinting and background checks
SAFE Certified *can be completed upon hire
