Job Summary:
Responsible for leading and managing all operations, business development, and strategic initiatives for home health and hospice services across a specific market or region. This role drives census growth, ensures high-quality patient care, maintains regulatory compliance, and achieves financial objectives. Expected to collaborate with senior leadership, clinical teams, referral sources, and community partners to expand market share, oversee multi-site operations, and uphold the organization’s mission of delivering compassionate, patient-centered care.
Essential Functions:
- Strategic Leadership: Develop and execute market-specific business plans to meet financial, operational, and clinical goals for both home health and hospice programs. Contribute to the organization’s overall strategic vision.
- Business Development: Lead sales and marketing strategies to increase patient census and market share. Build and maintain relationships with referral sources (e.g., hospitals, physicians, skilled nursing facilities) and community stakeholders. Conduct market analysis to identify growth opportunities and develop competitive strategies.
- Operational Oversight: Manage multi-site operations to ensure efficient delivery of home health and hospice services. Monitor key performance indicators such as patient outcomes, cost per case, and staff productivity. Ensure seamless coordination between home health and hospice programs.
- Team Leadership: Recruit, train, and manage clinical, sales, and operational teams. Foster a culture of collaboration, accountability, and excellence. Provide mentorship and professional development to ensure high-performing teams.
- Compliance and Quality: Ensure compliance with federal and state regulations, including Medicare and Medicaid standards, for both home health and hospice services. Oversee quality assurance programs to maintain CMS and CHAP accreditation and high patient satisfaction scores.
- Financial Management: Develop and manage budgets, revenue forecasts, and financial performance for the market. Oversee billing, reimbursement processes, and contract negotiations with managed care organizations. Optimize cost efficiencies while maintaining care quality.
- Stakeholder Engagement: Represent the organization to external partners, including healthcare providers, insurers, and regulatory bodies. Advocate for home health and hospice services within the community and promote early discussions about care options.

Minimum Position Qualifications:
Education: Bachelor’s degree in healthcare administration, business administration, or a related field required; Master’s degree (e.g., MBA, MHA) preferred.
Experience: 7-10+ years of progressive leadership experience in home health and/or hospice operations, with a focus on multi-site or multi-state management.
License / Certification: Driver’s license and proof of current auto liability insurance; no listing in the OIG Excluded Provider listing
Environmental Conditions:
Works under a variety of conditions in facilities and offices; ability to work flexible schedule; some exposure to unpleasant weather. moderate stress and emotional demands.
Physical Requirements: Sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Extensive travel within the market to oversee operations, meet with stakeholders, and support staff.
