    We are a mission-driven nonprofit dedicated to helping seniors and individuals with disabilities maintain consistent access to vital public benefits—most notably Medicaid home- and community-based services—while preserving their financial independence, dignity, and overall quality of life. Navigating Medicaid eligibility, particularly income limits, can be overwhelming and often forces individuals into difficult financial decisions that impact their ability to remain at home. Our Pooled Income Trust services are designed to provide a practical, compliant solution to this challenge. By allowing eligible individuals to shelter excess income, we help them avoid unnecessary spend-down requirements and continue affording essential living expenses—including housing, utilities, and home care costs—without jeopardizing their Medicaid eligibility. Beyond the technical solution, our work has a direct and meaningful impact: enabling individuals to age in place, maintain stability, and access the care they need in a familiar environment. We collaborate closely with a wide network of professionals, including elder law attorneys, geriatric care managers, social workers, discharge planners, home health agencies, and organizations supporting individuals with disabilities. Through these partnerships, we provide clear guidance, tailored support, and fully compliant solutions that make a tangible difference in the lives of vulnerable populations and the professionals who serve them.

    We are seeking a full-time Marketing / Sales Representative to drive the continued expansion of our referral network for Pooled Income Trust services. This is a highly field-based, relationship-driven role suited for someone who thrives in a fast-paced, independent environment and enjoys being out in the field building connections.

    This is a true “road warrior” position, where the majority of your time will be spent traveling and conducting in-person outreach across key regions including Long Island, all five boroughs of New York City, Westchester, and Rockland County. Success in this role comes from consistently being in front of people—meeting new referral sources, strengthening existing relationships, and becoming a trusted, go-to resource within the professional community. You will play a critical role in representing the organization externally, introducing new partners to our services, and helping them understand how we support their clients and patients. The ideal candidate is proactive, personable, and energized by face-to-face interaction, with a strong ability to build trust quickly and maintain long-term professional relationships.

    This role is open to candidates from a wide range of backgrounds—no prior experience in home care, Medicaid, or Pooled Income Trusts is required. We are looking for someone who is motivated, personable, and eager to build relationships in a face-to-face environment. If you have experience in healthcare, home care, or related fields, that is a strong plus, but not required.

    Key Responsibilities

    • Serve as the primary outreach and relationship manager for referral partners, including attorneys, social workers, care managers, and community organizations
    • Conduct frequent in-person visits, presentations, and follow-ups with prospective and existing partners
    • Build and maintain a pipeline through research, cold outreach, and networking
    • Learn and confidently explain our services and their benefits to referral sources
    • Attend networking events, conferences, and community meetings
    • Track outreach activity and referrals, and coordinate with internal teams
    • Represent the organization professionally with a client-first mindset

    Qualifications

    • No prior industry experience required
    • Background in sales, marketing, outreach, or client-facing roles is a plus
    • Strong communication and interpersonal skills
    • Comfortable learning and explaining new concepts
    • Self-motivated, organized, and able to work independently in a field-based role
    • Willingness to travel extensively within the assigned regions
    • Valid driver's license and reliable transportation

    Compensation

    • Base salary: $90,000 annually
