    SUMMARY:

    The Primary Care Nurse Practitioner (“NP”) is the leader of Matter Health in the communities we serve. The NP exists to provide exceptional care and patient experience through holistic, evidenced-based care. They will also support the Member Experience Manager in growing our member base as well as assist residents with the coordination of all of their health needs, social services and questions. Whether engaging directly with individuals in the community, organizing events to bring residents together, or working with community and clinical partners to coordinate and provide additional services, the NP is an active & visible member of the community. The NP combines traditional clinical skillsets with effective community engagement, problem solving, and organizational skills in a truly rewarding care role.

    Prior experience in clinic operations, care coordination or working with seniors is helpful; we are a startup healthcare provider open to great and diverse talent from many walks of life.

    JOB DESCRIPTION/DUTIES/ROLES:

    Patient focused quality and safety

    • Evaluate primary care, acute, chronically ill, and transitional care patient in addition providing healthcare education and counseling to residents
    • Provide patient counseling, medication management and education on health maintenance
    • Document all patient care within the Matter Health EMR
    • Provide care and coordination of our patients with internal and external colleagues, ensuring the highest standard of care is always provided for all patients
    • Work collaboratively with the patients care team – internal and external
    • Anticipate and fulfill the holistic needs of each patient
    • Phone triage outreach, patient education, population health leadership

    Patient Experience

    • Maintain an obsessive focus on the overall patient experience through the lens of how you would hope a loved one would be treated
    • Ensure we always get patients to an answer, regardless of the resources it takes
    • Go the extra mile to ensure we exceed expectations
    • Tailor your communication style to effectively influence quality outcomes and build relationships
    • Maintain patient confidentiality in accordance with PHI and HIPAA standards

    Community Building

    • Work closely with the Member Experience Manager to create a community in the development in which your clinic is located
    • Work to build personal relationships and social influence with all members of the community
    • Attend social events and become ingrained in the lives and well-being of each of the patients you serve
    • Engage with the community to create healthier social habits
    • Grow the Matter member base through different community events, 1:1 relationship building and other creative ways

    Autonomy & Accountability

    • Work independently in your clinic to prioritize and solve problems
    • Take initiative to improve your clinic while advocating for your patients
    • Actively participate in professional development opportunities and groups within your market (incl. regular knowledge shares with other staff across clinic locations)
    • Maintain self-awareness and professionalism of individual actions and how they impact the clinic, practice, and healthcare industry
    • Own the success of your practice through implementation of the clinic level business plan designed to meet identified business goals

    REQUIREMENTS

    • Passion for Matter Health concept, vision and patient population

    • Minimum Education: Master’s of Science in Nursing (MSN) Degree

    • Current Board Certification

    • Current State License to practice

    • Basic Life Support (BLS) certification

    • 5 -7+ years of work experience as Nurse Practitioner

      • Primary Care, Family Practice, or Geriatric Specialty Preferred
    • US Work Authorization Required

    • Prior experience with Athena (EMR) preferred

    • Ability to work without direct supervision and practice autonomously

    • Ability to collaborate with professional colleagues as necessary to provide quality care

    • Ability to consistently present a friendly, welcoming, and professional public-facing appearance

    • Outstanding organization + communication skills and ability to multi-task

    • Initiative, creativity, problem-solving ability, adaptability, and flexibility
