Responsible for scheduling skilled nursing and therapy field staff in the home health care agency
Responsible for intake of referrals from all hospitals, SNF’s & MD offices – Gate Keeper for Curaspan referrals
Responsible for demographic, referral entry, and patient welcome phone calls
Responsible for Hospital Hold Report – checking patients daily while on hold, ,retrieving ROC orders / processing post
hospital visits
Responsible for monitoring Alerts/Patient Not Seen Report daily
Responsible for adding new MD’S to the system
Responsible for creating Delta tickets for Scheduling/Intake issues
Qualifications /Training:
Must be competent to perform data entry and to work independently. Must be able to use computer for data entry
and report generation. Must have excellent organizational and project management skills
Preferred to have at least one year experience in home health or with scheduling multiple appointments on a daily
basis.
LICENSES/CERTIFICATIONS/REGISTRATIONS/EDUCATION:
Minimum of a High school diploma/Equivalent from an accredited school
